The doctors, nurse practitioners, and staff thank you for choosing Hunterdon Pediatric Associates to provide your medical care. You are now at an age where it is time for you to receive your medical care from a provider who treats adults. We want to make this transition as easy as possible for you.
Please see the list below of Hunterdon Health practices that care for adults. Please note: If you decide to see a Hunterdon Health provider, you will not need to have your records transferred since the new provider will have access to your records through our electronic health record. We will also provide you with a chart summary document that will include basic information about you including current medical problems, past medical history, and medications. If you have complex medical problems, and you are not already receiving help from our Care Coordinator, please call 908-806-8222 so she can help you navigate through the process of transitioning to an adult provider.
If you prefer, you may also choose an affiliated Hunterdon Health provider from our Physician Search.
If choosing a provider outside of the Hunterdon Health network, please complete a records release form and return it to the office after you have chosen your new medical practice.
