Important Message to Our Patients
As of January 1, 2025, our office will charge $10 to complete forms (examples, sports, camp, school, asthma action plans) outside of an office visit as long as your well visits are up to date. This fee is waived if the forms are presented at a well visit or if you are on Medicaid or the Patient Assistance Program.
